He knew only OKC players were in the vicinity, you could see him look slightly to the left as the elbow is being thrown, and, probably the most telling factor, after he makes contact, he keeps going and doesn't even look to see who he just decked. You don't hit someone that hard and not notice or turn around to see if the guy you just hit is ok.

As for his instability, some friends who know him from his St. John's days (where he worked out during his suspension) said that off the court he's a good guy, quiet and polite. But, between the lines he's a nutjob. For him the basketball court was his way out of the hood and something goes off inside of him when he steps on the floor.
